Summary:
The LVN/LPT is responsible to assist the with the coordination of the home so that the residents' health and safety needs are met, assists to maintaincompliance under the California Code of Regulations (CCR), Title 17 and Title 22 as well as the Lanterman Act (Welfare and Institutions Code).
The LVN/LPT isexpected to ensure residents to maximize independence.
JOB
Responsibilities:
Administer medications to individuals and follow the five rights for verification purposesAttend residents' doctor appointments and address any concerns and changes with the attending physicianAssist individuals with activities of daily living such as bathing, cleaning, preparing meals, grooming, cooking, cleaning etc.
Assess residents' and participants' wounds, help with dressing change and breathing treatments, as neededAudit all medications during a new medication cycle and check the five rights for verificationPerform g-tube insertions for feeding to individualsPerform colostomy care to individualsPerform intramuscular injectionsPerform diabetes insulin medication injection to individualsEvaluate residents' and participants' level of consciousness, neurological factors, and whether they are responsive and oriented post seizure and/or any traumatic episodesRedirect in the event of a behavioral episode and continue to support and listen to individual's needsComplete and update client notes system on a daily basis.
Observe and report any unusual behaviors and injuries so both Behavior Consultant and Administrator are fully informedAssess and identify sign and symptoms of infections (early and late signs)Perform basic physical review such as checking for vital signs, (understanding normal and abnormal measures) and head to toe assessmentsParticipate in community outing events with the individuals such as hiking, going to the beach, and other leisure activitiesPerforms other related duties and assignments as required Essential FunctionsOutline and prepare daily routine(s) for clients' (visual and active) as written in Individual Program Plan.
Assist clients with bathroom needs.
Change clients' undergarments and/or clothing if needed.
Prepare daily meals and assist with clean-up.
Pick-up/drop-off clients in company vans from/to their home/facility (if necessary).
Education and
Experience:
Direct Support Professional Certificate Year 1 & Year 2 (Preferred or willing to obtain within the first 6 months of hire)Must have First Aid/ CPR and CPI (Crisis Prevention Intervention)Must be at least 21 years of ageMust have a California Driver's LicenseCandidate must successfully pass all pre-employment requirements, including but not limited to a background checkPhysical RequirementsMust be able to lift 50lbs.
Able to work both indoors and outdoors in all weather conditions.
