Substance Abuse Counselor / Case Manager
Woodland Hills, CA Woodland Hills, CA Full-time Full-time $60,000 - $65,000 a year $60,000 - $65,000 a year Profound Treatment is looking to hire a certified counselor (CCAPP or CAADE) to serve as a counselor / case manager among our team of professionals within our substance use disorder treatment centers.
Duties & Responsibilities Oversees coordination of client needs and works in collaboration with client and primary therapist in treatment/discharge planning and goals, identifying additional needed support services including educational, vocational, legal, recreational, mental health and social services Completes and maintains all documentation of services delivered Ensures cohesion of services documented within client files, and assists with compliance in DHCS and Joint Commission standards Keeps a schedule of consistent case management sessions with clients Coordinates service provision with probation officers, attorneys, etc.
, as needed Complies with local, state, and federal confidentiality laws and regulations Facilitates group counseling sessions Collaborates with members of the treatment team to ensure effective delivery of a full spectrum of clinical and social services Attends and participates in weekly treatment team meetings Participates in staff meetings when appropriate Skills and Knowledge Required Must be able to accurately document all services provided to clients Ability to develop excellent rapport with clients Must be able to communicate effectively, both orally and in writing Must have the ability to coordinate a continuum of treatment / services for clients Education / Certification Requirements Must have a high school diploma or equivalent.
Must be certified with CAADE, CCAPP or CADTP.
